Direct Deposit - Direct deposit of payments from a gift annuity, pooled income fund, or charitable trust to your financial institution is a safe and time-saving alternative to receiving checks through the mail. Complete the Direct Deposit Authorization form to begin receiving this service.

IRS W-9 Form - Use this form if you are a recipient of income payments from trusts, pooled income funds, or gift annuities. You must provide the payor (the Foundation or NCTC) your correct Taxpayer Identification Number (TIN) for proper tax reporting, as required by law.

Gift Agreements:

You can make a gift now.  Follow the links below to obtain a Gift Agreement. Simply print the agreement, complete the information needed and submit with your gift to the Foundation.  If you would like an agreement faxed or mailed to you, please call Client Services at 800-858-6127 or e-mail us.

After your gift is accepted by the Foundation, you will receive an acknowledgement that your gift has been accepted and a summary of the contract. Questions concerning the completion of a gift agreement can be directed to a client service representative at 800-858-6127.
