Personal Information Change Form
Use this form to notify us of any changes in your address, phone number or other contact information.
Direct Deposit Authorization Form
Direct deposit of payments from a gift annuity, pooled income fund, or charitable trust to your financial institution is a safe and time-saving alternative to receiving checks through the mail.
IRS W-9 Form
Use the W-9 form if you are a recipient of income payments from trusts, pooled income funds, or gift annuities. You must provide the payor (the Foundation or NCTC) your correct Taxpayer Identification Number (TIN) for proper tax reporting, as required by law.
PC(USA) Relatedness Form
For charitable beneficiaries that are not part of a governing body of the Presbyterian Church (USA), but are considered a validated mission, we require additional information to process the gift and to confirm the ongoing relationship of the organization to the Presbyterian Church (USA). Please submit this form to firstname.lastname@example.org.
Trust Revocation Form
This Trust Revocation Notification must be completed if the Gift assets are titled to a revocable trust.
Gift Agreement Form
You can make a gift now. Simply print the agreement, complete the information needed and submit with your gift to the Foundation. You will receive an acknowledgment once your gift is accepted by the Foundation, along with a summary of the contract.
